Facility: I understand training at Gracie Barra Southlake (GBSL LLC) involves use of mats, equipment, and shared spaces. I will follow staff direction regarding facility use and safety.
Photo & Video: I authorize Gracie Barra Southlake (GBSL LLC) to use photos/video of me (or my child) for academy marketing and educational purposes.
Physical Condition: I certify that I (or my child) am physically able to participate in Brazilian Jiu-Jitsu and related activities, and I will inform instructors of any relevant medical conditions.
Mat Etiquette: I agree to follow academy rules, respect coaches and teammates, maintain hygiene, wear appropriate attire, and train safely.
GENERAL RELEASE: In consideration of being allowed to participate in classes, open mat, and related activities at Gracie Barra Southlake (GBSL LLC), I release and hold harmless GBSL LLC, its owners, instructors, employees, agents, and the premises owners/lessors from claims arising out of ordinary negligence related to my (or my child’s) participation, to the fullest extent permitted by law. I understand BJJ involves inherent risk of injury. I have read this agreement, understand I am giving up substantial rights including the right to sue, and sign freely.
Uniform return: I acknowledge that any loaned academy uniform/gear must be returned clean and in good condition, or I may be charged for replacement.
